What is the Deep Plane Facelift?
It is the facial rejuvenation technique that works on the deep plane of the face: the ligaments and the muscle-aponeurotic layer (SMAS) where aging really happens. Instead of tightening the skin, it repositions the tissues to their original place.
That is why the face recovers the volume and definition it had 10 or 15 years ago, without the "pulled" look of conventional facelifts. People around you notice you look rested, but they cannot tell what changed.
Dr. Carlos Andrés Arango was certified in March 2024 in the course "The New Frontiers of Face Lifting" at the Plastic Surgery Academy in Rome, with Dr. Michele Pascali.
Deep Plane vs. conventional facelift
| Conventional facelift | Deep Plane | |
|---|---|---|
| Where it works | Skin and superficial layer | Ligaments and deep plane |
| Duration | 3 to 5 years | 10 to 15 years |
| Look | Risk of tight or "pulled" skin | Natural, with no tension on the skin |
| Midface | Limited correction | Restores cheekbones and folds |
| Neck | Separate procedure | Treated in the same surgery |
Areas it improves
- Midface: restores volume and projection to the cheekbones.
- Nasolabial folds: softens them by repositioning tissue, not by pulling skin.
- Jawline: redefines the oval of the face.
- Neck: double chin, bands and sagging in the same surgery (it can be combined with a neck lift).
- Expression: stays natural because the muscles are not left tight.
Is it for you?
It is usually recommended between ages 45 and 65, when the jawline loses definition, the nasolabial fold deepens and the neck sags, and non-surgical treatments are no longer enough.
It may not be the best option if you are under 40, if the problem is only volume (which can be solved without surgery) or if you smoke and cannot stop. At the consultation the doctor will tell you honestly.

Frequently asked questions
What people ask us most about the Deep Plane
How much does a Deep Plane Facelift cost in Medellín?
The cost depends on whether it is combined with the neck, eyelids or other procedures. You receive an exact quote after the consultation, which costs $100.000 COP in Colombia.
How long does the result last?
Between 10 and 15 years, because it works on the deep tissues where aging happens. Superficial facelifts usually last 3 to 5 years.
Will I look operated on or pulled?
No. In the Deep Plane the tension is spread across the deep layers and the skin is repositioned without pulling. You look like yourself 10 or 15 years ago.
What is the best age to have it?
Usually between 45 and 65, when sagging of the jawline and neck no longer responds to non-surgical treatments.
When can I return to my social life?
Between 3 and 4 weeks most patients are presentable and back at work. The final result is seen between 3 and 6 months.
Can it be combined with other procedures?
Yes. It is often combined with blepharoplasty, a neck lift or a lip lift in the same surgery, when it is safe to do so.
